Minor Photo Release

Photos and videos may occasionally be taken for use on ForteForest.com and in other promotional materials.  You will be asked to sign a photo release at check-in.  If you choose not to sign the photo release form, you child’s image will not be used.  The photo release statement reads:

I hereby consent to and authorize Kortney Ross to use photographs and/or video which have been taken of my child during Camp Granada by herself or an agent representing Forte Forest for reproduction in brochures, booklets, print advertisements and/or on the web for promotional purposes. The associated files/pictures are the property of Kortney Ross to be used only as described above. In the event that your child’s likeness is used, there will be no identifying information.

Participation Agreement

When registering you child for Camp Granada through Forte Forest, you will be asked to sign an agreement to ensure your consent to the following liability statement:

In consideration of my child’s participation in Camp Granada through Forte Forest at the Grove Theater facility, on behalf of myself, my family, my heirs, and my assigns, I hereby release and hold harmless Mrs. Kortney Ross and any agents or employees of Forte Forest, High Places Community Church, Friends of the Grove Theater, and other participants from liability for injury, death, property damage/loss, or any other claim actionable in a court of law suffered by myself or my child while participating in Camp Granada activities.

Activities include, but are not limited to, singing, listening, playing instruments, movement activities, and the use of musical equipment and movement props.  I acknowledge that I understand and appreciate the inherent risks of participating in these activities as a part of Camp Granada.  I know the risks range from minor scrapes, strains, and bruises to significant injuries such as broken bones, eye injury, concussion, paralysis, and even death, and may result from mine or my child’s own actions, the actions of others, or a combination of both. By signing this agreement, I fully assume the inherent risks associated with the use of the Grove Theater facilities and the equipment and activities provided by Mrs. Kortney Ross or her agents for the camp and assert that my child’s participation in them is voluntary.

I hereby certify that I have adequate health insurance to cover any injury or damages that I/my child may suffer while participating, or alternatively, agree to bear all costs associated with any such injury or damages to myself/my child.
